Terms and conditions

The Agreement
  1. The general terms and conditions form an integral part of the treatment agreement(s) between the oral-care provider (hereinafter: the “care provider”), including its employees, and the patient. They are made available to the patient or their legal representative(s) before or at the conclusion of the agreement(s), are present in the waiting room, and will be sent upon request.
Identification
  1. Everyone must be able to identify themselves at all times with a valid ID and insurance card, to prevent misuse and fraud of your patient file and insurance. This also applies to persons under 14 years of age.
Third Parties
  1. The care provider is entitled to engage third parties in the execution of the agreement. He will exercise due care in selecting these third parties and, if reasonably possible, consult the patient beforehand, except in the case of substitutes and dental technicians. The care provider is not liable for the conduct of these third parties. Articles 7:404, 7:409 and 7:422 paragraph 1 of the Dutch Civil Code do not apply.
Costs
  1. The treatment costs, including technical and material costs, will be charged to and are payable by the patient, regardless of whether these costs can be wholly or partly claimed from, and reimbursed to the patient by, a health insurer. Treatment costs for minors will be charged to and are payable by their legal representative(s).
  1. Unless indicated otherwise, the dentist calculates the treatment costs according to the Dental Care Tariff List and General Provisions as approved or established by the Dutch Healthcare Authority (NZa). Changes to the Dental Care Tariff List and General Provisions are reserved.
Appointments
  1. Appointments must be cancelled no later than 24 hours before the treatment. In case of late or no cancellation, the care provider is entitled to charge the reserved time and/or the scheduled treatment if no other patients could be treated or other compensated work could be carried out.
  2. In case of repeatedly missing your appointment without explanation, we reserve the right to deregister you as a patient.
  1. If you have not attended a periodic check-up for more than 2 years,
we reserve the right to deregister you as a patient. Treatment and Information
  1. The care provider informs the patient about the treatment plan, possible risks, and other potential treatment options, so that the patient can make a well-considered decision. The information is provided at a level appropriate for the patient, and the care provider verifies that the patient has understood the information.
  1. Before any extensive and/or invasive dental treatment that entails costs of €250 (two hundred fifty euros) or more, the patient or their legal representative will receive a quotation in writing and/or digitally. This quotation provides an overview of the intended procedures, the tariff per procedure, and the estimated material and technical costs.
  2. 11. If providing a written quotation beforehand is reasonably not possible, the care provider is not required to do so. In that case, the care provider will discuss the treatment costs with the patient beforehand, unless that is also reasonably not possible.
  1. The care provider may deviate from the agreed treatment plan and the above-mentioned quotation if, during examination or treatment, deviation is found to be necessary due to unforeseen complications. The care provider will inform the patient of the deviation and any cost overrun, unless this is reasonably not possible.
  1. The patient is required to provide all information and documents that the care provider deems necessary for the execution of the treatment agreement, in a timely manner and in the requested form and manner. Extra costs arising from not, not timely, improperly or incompletely providing the required information will be borne by the patient.
  1. The patient guarantees the accuracy, completeness, and reliability of the information and documents provided to the care provider.
  1. At the patient’s request, the care provider will record in writing which procedures, whether invasive or not, the patient has consented to.
  1. It is not permitted to bring pets into the practice. An exception is made for assistance dogs required due to a disability.
  2. In case of rude, indecent, intimidating, or aggressive behavior toward the practitioner or any other staff or patients within our practice, we reserve the right to immediately deregister you as a patient.
Billing
  1. Billing is handled through the factoring company Infomedics.

  1. If invoices remain unpaid, treatment may be suspended
until all outstanding invoices have been settled. Complaints

  1. The liability of the care provider is limited to the amount covered by the professional liability insurance taken out by the care provider, plus the amount of the care provider’s deductible under the insurance. The patient may inspect this policy at the care provider’s practice upon request.
